AccentCare, Inc. is seeking a Hospice Care Consultant in Saint Paul, MN to drive hospice sales, educate referral sources, and ensure timely, quality patient care. You will build trusted relationships with physicians, hospitals, and community partners.

This full-time role offers a salary range of $70,000 to $110,000 plus monthly bonuses and mileage, with comprehensive benefits including medical, dental, PTO, 401(k) match, and professional development.

Formación

• Bachelor’s degree or 5 years of proven experience in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory effectively.

Responsabilidades

• Develop and maintain relationships with physicians, hospitals, SNFs and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships and identifying new referral opportunities.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and execute community outreach plans.
• Partner with clinical staff to ensure referrals transition smoothly into care, prioritizing patient comfort and dignity.
• Uphold commitments to quality, ethics, and confidentiality through training and outreach planning.
