Job summary

HopeHealth seeks a Clinical Coordinator to support our hospice nursing team in Wakefield, RI, with some coverage in Providence or Attleboro. The role is full-time, 7am-3:30pm, reporting to the Clinical management team.

You’ll coordinate patient care, schedule visits, and help manage EMR data to improve performance and patient outcomes. Ideal candidates hold an LPN or therapy assistant license in RI/MA and have at least two years of clinical experience.

Qualifications

• Licensed Practical Nurse or Therapy Assistant.
• Licensed as an LPN or therapy assistant in Rhode Island and Massachusetts.
• Minimum of 2 years of clinical experience in healthcare (home care, hospital or hospice).
• Excellent assessment, clinical and communication skills.

Responsibilities

• Works closely with Patient Clinical Manager and Hospice Service Coordinators.
• Supports patients, families, and clinicians.
• Scheduling of staff visits per the patient care plan as needed.
• Utilizes reports and EMR features to monitor, track and improve clinical, quality and financial performance metrics.
• Monitors field staff caseloads and ensure field staff scheduling visits meet regulatory timepoint requirements.
• Follow up with patients/families and facilities as directed by the Patient Clinical Manager.
• Assumes responsibility for personal growth and development; attends mandatory in-services.
• Adheres to organizational values, HIPAA compliance and safety policies.
• Other duties as assigned.
