Job summary

HopeHealth is seeking a part-time Licensed Practical Nurse (LPN) to join our Rhode Island team in Wakefield. The role supports home and nursing home patients with hospice care, under the guidance of the nurse case manager and attending physician.

This 32-hour week position runs 8:00am–4:30pm across five days with a rotating weekend commitment. The ideal candidate holds an active MA license and strong nursing assessment skills, and is committed to compassionate, patient-centered care.

Qualifications

• Current MA Nursing License.
• Two years hospice or oncology preferred.
• Must have well developed nursing assessment and clinical skills.
• LPN required.
• Hospice/Palliative care nursing certification preferred.

Responsibilities

• Conducts assessments for home and nursing home patients.
• Implements the plan of care under the direction of the nurse case manager and attending physician.
• Instructs the caregiver in the care of the terminally ill based on individual differences and education.
• Demonstrates skill in interpersonal relationships with Hospice patients, families, nursing home staff, co-workers and community support services.
• Maintains accurate records of observations, problems and nursing actions.
