Job summary

VITAS Healthcare Corporation in Naples, FL is seeking an experienced Registered Nurse (RN) to coordinate care for hospice patients and their families, and to implement the interdisciplinary plan of care with attention to physical, psychosocial, spiritual and bereavement needs.

You will perform comprehensive assessments, supervise nursing tasks, educate families, and participate in team meetings to ensure high-quality end-of-life care.

Qualifications

• Current license to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Follows VITAS policies and standards and practices within the scope of the RN license
• Reports any request to practice outside of scope of license to the manager/supervisor
• Performs supervision of nursing tasks delegated to Hospice Aides, LPNs, HVNs and Volunteers
• Completes initial and ongoing comprehensive assessments to provide palliation of end-of-life symptoms
• Identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment
• Completes documentation at the time of the event and at the location of care
• Provides education and training to patients, families, caregivers and referral sources
• Participates in patient care meetings and weekly team meetings
• Provides effective communication/shift reports and patient/family advocacy
• Participates in bereavement support and on-call rotation
• Delivers care in accordance with the patient’s plan of care and coordinates with IDG
• Evaluates outcomes of services and delegates tasks appropriately
• Represents the nursing profession and VITAS positively
• Ensures safety factors and compliance with standards
