Job summary

Community Healthcare of Texas is seeking a Care Coordinator in Fort Worth, TX to professionally coordinate hospice services and bridge patient transitions. You will build long-term relationships with facilities to position CHOT/Providence Hospice as the provider of choice.

Responsibilities include responding to referrals, coordinating paperwork and discharges, educating staff, and contributing to increasing hospital referralsthrough strong service and collaboration.

Qualifications

• 1–3 years of healthcare marketing experience required.
• 1 year of hospice experience required.
• Medical knowledge strongly preferred (ability to read charts, know medications, etc.).
• Proficient using PC including Microsoft Office.

Responsibilities

• Respond to referral sources in a timely manner; evaluate patient desire for hospice; meet with families/patients to discuss goals and explain services of hospice.
• Complete required paperwork with patients/families; facilitate patient discharge by coordinating with facility staff, DME, medications, hospice physicians, and other staff.
• Educate facility/hospital staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
• Communicate and provide daily feedback to admission staff on referrals.
• Through customer service and leveraging relationships, become the hospice of choice, increasing hospital referrals.
• Attend and participate in routine staff meetings in person or via media provided as directed.
• Participate in or provide in-service training, attend various facility/hospital activities and perform other duties as required or assigned.
