Job summary

VNS Health is seeking a Hospice Operations Coordinator to support the Hospice Care team. The role coordinates communications, scheduling, and documentation to ensure high-quality patient care and seamless operations.

The position partners with the Team Manager, IDG, and Intake teams to manage patient admissions, transitions, and care coordination, while coordinating DME and vendor requests for timely delivery.

Qualifications

• Associate's Degree in Healthcare preferred or HS diploma with relevant experience.
• 2+ years administrative or customer service experience in healthcare/hospice.
• Strong communication and detail orientation; able to multitask in a fast-paced setting.

Responsibilities

• Serve as primary administrative liaison for the hospice team and maintain clear communication with clinicians, patients, families, and vendors.
• Coordinate scheduling of hospice visits per physician orders and ensure timely delivery of DME, pharmacy, and supplies.
• Support admissions, transitions, and care coordination with IDG, Team Manager, and Intake team.
• Prepare, facilitate, and close IDG meetings to meet regulatory requirements.
• Monitor documentation for completeness and escalate missing items.
• Assist with shift change reports for After Hours leadership as needed.
• Maintain organized schedules, logs, and workflows to optimize operations.
