Job summary

Compassus in Michigan is seeking a Hospice Team Coordinator to oversee care delivery, maintain patient records, and support collaboration among the interdisciplinary team. You will manage referral intake and admission documents, upload to HCHB, verify billing details, and ensure timely communication with physicians, nurses, and families to provide compassionate, compliant care.

Strong communication and accuracy are essential, with a high school diploma and at least two years’ medical records

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records or medical office support.
• Strong written and verbal communications.
• Ability to manage patient data and scheduling with sensitivity to confidentiality.

Responsibilities

• Enter and manage patient data including referrals and demographics.
• Ensure complete admission documents with physician CTI and consent.
• Upload documents into HCHB and maintain records.
• Use HCHB to generate reports and coordinate care notes.
• Monitor DME orders and inform management of trends.
• Verify and upload paper visit records into EMR/HCHB.
• Coordinate with IDT for holistic patient care.
• Serve as central contact for hospice team, patients, and families.
• Handle calls and manage the phone system.
• Assist DCS with IDT prep and updating plans of care.
• Prepare patient binders and aid discharge communications.
