Providence at Home with Compassus in Portland, OR is seeking a Hospice Team Coordinator to orchestrate hospice care delivery, maintain patient records, and facilitate collaboration among the care team to ensure compassionate, high-quality support for patients and families.

The role demands meticulous data handling, proficiency with EMR/HCHB systems, and effective communication with physicians, nurses, and other stakeholders.

Competenze

• Math: basic arithmetic, rates, ratios, percentages.
• English: reading, interpreting procedures and regulations.
• Strong written and verbal communication abilities.

Mansioni

• Accurately enters and manages various types of patient data and updates demographics.
• Ensures admission docs are complete with CTI and order to admit; uploads to HCHB.
• Uses HCHB to maintain records and generate regulatory reports.
• Monitors DME orders and informs management of trends.
• Verifies and uploads paper visit records into EMR and HCHB.
• Organizes documents in patient charts for easy retrieval.
• Ensures care level matches billing level for each patient.
• Coordinates with IDT to ensure holistic patient care.
• Acts as central point of contact for patients and families; handles calls.
• Supports DCS with pre/post-IDT activities and updates plans of care.
• Prepares binders for patients in facilities; performs other duties as assigned.
