Providence at Home with Compassus is seeking a Hospice Team Coordinator to oversee data entry, record accuracy, and interdisciplinary team coordination for high-quality hospice care. The role emphasizes strict documentation, regulatory compliance, and seamless communication with patients and families.

Responsibilities include managing admissions, using HCHB/EMR systems, ensuring proper billing alignment, and supporting the Hospice Director of Clinical Services with IDT activities and care plans.

Qualifications

• High school diploma or GED required.
• At least two (2) years of experience with medical records, medical office, or business office support highly preferred.

Responsabilités

• Accurately enters and manages various patient data, including referral intake information and demographics.
• Ensures all admission documentation is complete, signed, dated, and includes CTI and order to admit; uploads documents into HCHB.
• Uses HCHB to maintain and update patient records and generate regulatory reports; adds coordination notes.
• Monitors DME and medical supplies orders and informs management of trends or issues.
• Verifies and uploads paper visit records into EMR and HCHB for complete histories.
• Ensures organized document filing within patient charts for easy retrieval.
• Confirms that level of care matches billable level of care for accurate billing.
• Collects decertification paperwork and notifies providers of patient discharge via coordination notes.
