Job summary

Compassus is seeking a Social Services Coordinator to promote the Compassus philosophy and collaborate with the LMSW-led hospice team to improve quality of life for patients at end of life. You will follow the Plan of Care, coordinate community resources, and communicate patient needs to the supervising LMSW.

The role emphasizes advocacy for patients and families, education about social services, and maintaining thorough documentation within the care plan.

Qualifications

• Bachelor’s degree in Social Work from an institution accredited by the Council on Social Work Education, or a Bachelor's degree in psychology, counseling, or related field.
• One (1) year of social work in a healthcare setting.
• Previous hospice, palliative care, or healthcare experience preferred.

Responsibilities

• Follows the established Plan of Care providing support to meet the emotional, social, and practical needs of patients and families.
• Collaborates with the hospice team to support the development of individualized care plans tailored to the unique needs of each patient.
• Provides emotional support to patients and their families, fostering a compassionate, and understanding environment.
• Assists individuals in coping with grief, loss, and the emotional challenges associated with end-of-life care.
