Gentiva Hospice is seeking a Hospice Care Consultant to drive referral development and educate healthcare partners about hospice options. You will build and maintain relationships with hospitals, home health agencies, SNFs, and physician offices to ensure timely referrals and improve patient access to quality end-of-life care.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitivity to needs of terminally ill patients and families.
• Proven track record of meeting admissions and census targets.
