Job summary

Hospice of the Chesapeake Inc. is seeking a Care Access Coordinator in Pasadena, MD to bridge access to care by coordinating referrals, admissions, and liaison visits. The role oversees DME orders and ensures timely communication with physicians and facilities.

The position requires a high school diploma (associate degree preferred) and strong customer service skills, with experience in hospice or home health care being a plus. Monday–Friday, with competitive pay and benefits.

Qualifications

• High school diploma or equivalent required; an associate degree in a related field preferred.
• At least 1 year of experience in hospice, home healthcare, or a customer service role is preferred.
• Demonstrate exceptional customer service and understanding of hospice philosophy.
• Ability to work independently and in a team with strong multitasking and deadlines.
• Effective communication with medical professionals across disciplines and strong computer skills.

Responsibilities

• Respond to referral calls and complete associated paperwork and records.
• Explain hospice services to callers and visitors as needed.
• Order DME, track and document appropriately.
• Coordinate with DME company, attending physician, and care facilities to track admissions.
• For Medicare patients, verify eligibility dates and prior hospice benefits.
• Enter referral information in electronic medical record software.
• Collaborate with finance to ensure proper billing of services.
• Schedule admissions visits.
