The role emphasizes exceptional service to referral sources, patients, and families, with a focus on timely information and accurate processing.

The candidate should have strong customer service, multitasking, and communication skills, with experience in hospice/home health or related fields.

Qualifications

• High school diploma or equivalent required; an associate degree in a related field preferred.
• Candidates with commensurate hospice/home care or customer service experience will be considered.

Responsibilities

• Respond to referrals and complete related paperwork and records.
• Explain hospice services to callers and visitors when needed.
• Order and track DME, document appropriately.
• Coordinate communication with DME company, attending physicians, and care facilities.
• Verify Medicare eligibility dates and hospice benefits for patients.
• Enter referral information into electronic medical records system.
• Assist finance with billing hospice services.
• Schedule admissions visits.
