Job summary

Heart’n Soul Hospice is seeking a dedicated Hospice Aide (CNA) to join our team. You will provide direct patient care under RN supervision and follow our Plan of Care to ensure compassionate, high-quality end-of-life services.

Responsibilities include safe transfers, documentation, infection control, and clear communication with patients and families. A valid CNA license, high school diploma or GED, and at least one year of hospice/nursing assistant experience are required.

Qualifications

• High School Diploma or GED (preferred)
• Current CNA license (unencumbered)
• At least one year experience as Hospice Aide or Nursing Assistant
• EMR experience a plus

• Provide direct patient care to patients under the RN and Plan of Care
• Assist with safe transfers and ambulation per policy
• Document pertinent information and care to ensure continuity of care
• Follow infection control principles
• Promote respectful communication with patient/family and staff
• Timely submission of all documentation
• Perform other duties as required
• Ensure compliance with local, state and federal laws and Medicare regulations

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
