Job summary

Heart’n Soul Hospice in Murfreesboro, TN is seeking a qualified Social Worker to join our compassionate team. The role focuses on psychosocial assessment, counseling, and coordinating resources for patients and families facing life-limiting illness.

Required: Bachelor’s or Master’s in social work, active state license, valid driver’s license with reliable transportation, and at least one year of clinical social work experience (hospice preferred).

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting required; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with advance directives, funeral planning issues and transfer of responsibilities.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet patient needs.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure compliance with local, state and federal laws, Medicare regulations and policies.

What You Will Do:

• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
