Job summary

St. Croix Hospice is seeking a dedicated Hospice Aide (CNA) to deliver compassionate, in-home care under RN supervision. You will assist patients with daily living activities, monitor vital signs, and help maintain comfort and dignity at the end of life.

Responsibilities include personal care, meal assistance, and accurate documentation. The role requires travel to patient homes across the schedule and a valid CNA credential in Wisconsin.

Qualifications

• Current CNA certification in the state of practice.
• Valid driver’s license, clean driving record, and auto insurance.
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.

Responsibilities

• Deliver compassionate, hands-on care to patients in their homes.
• Support patients with personal care and daily living activities to promote comfort and dignity.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Travel to patient homes throughout the day and provide care as scheduled.
