Job summary

Saint Agnes Home Health and Hospice is seeking a Home Health Aide to provide one-to-one care in patients' homes, following physician orders and complying with California regulations.

The role emphasizes patient-centered care, professional development, and flexible scheduling within a Catholic-based, non-profit home health system that is part of Trinity Health At Home. A valid CA HHA certification and a current driver's license are required.

Qualifications

• Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
• Must be certified as HHA in state of California
• Documentation of one year's employment as an aide in an institution or home care setting.
• Meets Saint Agnes Home Health and Hospice's hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
• Must accept responsibility for maintaining skills, learning and adhering to agency's policies and procedures on an on-going basis.
• Must have current driver's license and reliable transportation to and from work site

Home Health Aide position summary - Hospice

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
