Job summary

Saint Agnes Home Health and Hospice in California seeks a Hospice Admissions RN for the 2pm-11pm shift. This role will initiate hospice plans of care and ensure timely communication, while delivering compassionate, patient-centered home health care.

Requirements include California RN licensure with at least 1 year of nursing experience; BSN preferred. Sign-on bonus of $10,000 and a strong benefits package are offered.

What You’ll Do

• Provide skilled nursing care in accordance with physician orders and organizational policies
• Perform comprehensive assessments and develop individualized care plansCollaborate with physicians, therapists, social workers, and home health aides
• Supervise and support LPNs and Home Health Aides
• Ensure compliance with regulatory standards and contribute to quality improvement initiatives
• Promote patient satisfaction and positive health outcomes

What You Bring

• Current RN license (BSN preferred)
• Minimum 1 year of nursing experience required in California
• Home healthcare experience preferred
• Valid driver’s license and reliable transportation
• Strong computer skills and a commitment to ethical, mission-driven care

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
