Job summary

Saint Agnes Home Health and Hospice, part of Trinity Health At Home, seeks a dedicated Home Health Aide to provide compassionate one-to-one care in patients' homes. The role requires CA HHA certification, a valid driver's license, and the ability to follow physician orders and agency policies.

You will work in a patient-centered environment with supportive colleagues, access to advanced remote monitoring technology, and opportunities for professional development.

Qualifications

• Must be certified as HHA in California.
• Minimum of one year's employment as an aide in an institution or home care setting.
• Completion of federally compliant training program as required by law/regulation and competency evaluation.
• Must have current driver's license and reliable transportation to worksite.
• Ability to function with minimal direct supervision and adhere to agency policies.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Follow physician orders and comply with Nurse Practice Act and licensure requirements.
• Utilize the agency's policies and procedures and maintain professional relationships with clients and families.
• Engage with the care team and use available remote monitoring technology where applicable.
• Maintain skills and participate in ongoing training and orientation.

Home Health Aide position summary – Hospice

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

Minimum qualifications

• Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
• Must be certified as HHA in state of California
• Documentation of one year's employment as an aide in an institution or home care setting.
• Meets Saint Agnes Home Health and Hospice's hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
• Must accept responsibility for maintaining skills, learning and adhering to agency's policies and procedures on an on-going basis.
• Must have current driver's license and reliable transportation to and from work site
