Job summary

AccentCare, Inc. is seeking a Be the Best Hospice After Hours Admission RN You Can Be to join our on-call team in the Baltimore area. You will perform assessments, develop care plans with the IDG, and coordinate admissions after hours.

The role requires RN licensure in the applicable state, and experience in hospice or home health. You will work a Monday-Thursday 5:00pm-8:00am schedule with on-call responsibilities and travel within the coverage area.

Qualifications

• Registered Nurse license in the applicable state is required.
• Associate degree in nursing preferred; CPR certification recommended (where applicable).
• Experience in hospice, home health, medical-surgical, or geriatrics/pediatrics is preferred.
• Willingness to participate in on-call rotation and reliable transportation with a valid driver's license and insurance.
• Strong communication and organizational skills; ability to collaborate with an interdisciplinary team.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the IDG to develop and evaluate individualized plans of care.
• Maintain timely, accurate documentation and ensure care plans reflect current patient status.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations, admissions, and care transitions.
• If Care Manager: coordinate all aspects of patient care including scheduling, medication management, and supervisor roles.
• If On‑Call: respond to triage calls, perform visits, coordinate care with administrative On-Call and IDG meetings.
