Job summary

Mercy Health Home Care and Hospice by Compassus in Kentucky is seeking a Hospice Admissions Coordinator to manage referrals, coordinate admissions, and input data after all hospice requirements are satisfied. An RN is preferred, with other licensed clinical education considered.

The role requires 2–3 years of related experience, strong communication with physicians, nurses, and healthcare staff, and familiarity with insurance billing and Medicare/Medicaid processes.

Qualifications

• High school diploma or GED required; Bachelor's degree preferred.
• RN preferred; other clinical licensure strongly considered.
• 2–3 years of related experience strongly preferred; experience with physicians and healthcare staff is highly valued.
• Experience with insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Process patient referrals and coordinate admission of patients.
• Enter patient admission data after all hospice requirements are met.
• Read charts, identify needed documentation, and communicate with supervisors.
• Contact physicians to secure admission orders and arrange RN assessments.
• Provide information about hospice services to patients and families; liaise with IDT and providers.
