Job summary

Compassus seeks a Hospice Admissions Coordinator to manage referrals, coordinate patient admissions, and enter admission data following hospice requirements. You will receive referrals from physicians, families, and community, contact physicians for admission orders, schedule consent and RN assessments, and ensure accurate charting and data entry in a third-party system.

Qualifications

• High school diploma or GED required; bachelor's preferred.
• 2–3 years of related experience strongly preferred.
• Experience with physicians, nurses, and healthcare staff preferred.

Responsibilities

• Receive and process referrals from physicians, patients, families, and community.
• Contact physicians to secure admission orders.
• Schedule patient and family appointments for informed consents and RN assessment.
• Enter admission data into a third-party system.
• Notify IDT and ancillary providers of admissions.
