Job summary

Compassus is seeking a Hospice Admissions Coordinator to process patient referrals, coordinate admissions, and enter data after hospice requirements are met. This is a local, non-remote role with hours Monday–Friday 8am–5pm; LPN preferred.

The role requires communicating with physicians, scheduling visits, explaining hospice services, and ensuring proper documentation and IDT notification. Experience in hospice and billing is a plus.

Qualifications

• High school diploma or GED required; Bachelor's preferred.
• LPN preferred; two to three years in related experience preferred.
• Experience communicating with physicians, nurses and healthcare staff preferred.

Responsibilities

• Process referrals from physicians, patients, families, and others.
• Coordinate patient admission and schedule RN assessments.
• Enter admission data into third‑party systems and ensure chart completeness.
• Explain hospice services and benefits to patients and families.
• Notify IDT and ancillary providers of admissions and appointments.
• Market referral sources and provide back-up support as needed.
