Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission.
• Outline the patient/family referral and admission.
• Use assessment data to determine care plan needs and initiate the plan of care.
• Articulates hospice philosophy, goals and objectives.
• Communicates with other members of the clinical team to coordinate the care plan.
• Markets to acute hospitals in the area.
