Phoenix Hospice in Kansas City, MO is seeking an Admissions RN to guide patients and families from curative to comfort-focused care. You’ll collaborate with intake coordinators, medical directors, and facility staff to ensure smooth admissions with compassion and clinical excellence.

Qualifications

• Hospice experience required.
• Active Missouri or Compact RN license.
• Strong assessment, organization, and communication skills.
• Ability to work independently while collaborating with a team.
• Valid driver’s license.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Build meaningful, day-to-day connections with patients and families.
• Provide education on conditions such as CHF, COPD, and Diabetes.
• Perform wound care and medication management.
• Change dressings and medical devices as ordered.
• Accurately document patient conditions and communicate changes to physicians.
• Partner closely with interdisciplinary hospice teams to deliver seamless care.
