Job summary

Phoenix Hospice is seeking an Admissions RN to guide patients and families from curative to comfort-focused care with compassion and clinical excellence. You will lead admissions visits and collaborate with intake coordinators and facility staff. Active Missouri or Compact RN license is required.

Hospice experience, strong assessment and communication skills, and the ability to work independently while collaborating with a team are essential. Valid driver's license and reliable transportation.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Build meaningful, day‑to‑day connections with patients and families.
• Provide education on conditions such as CHF, COPD, and Diabetes.
• Perform wound care and medication management.
• Change dressings and medical devices as ordered.
• Accurately document patient conditions and communicate changes to physicians.
• Partner closely with interdisciplinary hospice teams to deliver seamless care.

You'll collaborate closely with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure every admission is smooth, thoughtful, and centered on what matters most.
