Job summary

Phoenix Home Care & Hospice in Kansas City, MO is seeking a Hospice Admissions RN for a full-time, 40-hour week with 10-hour shifts, Monday through Friday. You will be the calm first contact guiding patients and families from curative to comfort-focused care with compassion and clinical excellence.

You’ll collaborate with intake coordinators, medical directors, and facility staff to ensure admissions are smooth and centered on what matters most.

Qualifications

• Hospice experience required.
• Active Missouri or Compact RN license.
• Valid driver’s license.
• Strong assessment, organization, and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Provide compassionate care and support to patients and families.
• Educate patients and families on conditions such as CHF, COPD, and diabetes.
• Provide wound care and medication management.
• Document patient conditions and communicate changes to physicians.
• Collaborate with the interdisciplinary hospice team.
