Job summary

Ascend Hospice KS is seeking an Admissions Nurse to join our team in the KSMO area. The role focuses on initial patient/family assessments, admission planning, and care coordination to ensure timely and compassionate home-based hospice care.

The position requires an RN license with 2-4 years of hospice experience. Associate degree is required, Bachelor’s degree is preferred, and the candidate will collaborate with a multidisciplinary team to implement individualized care plans and promote

Responsibilities

Admissions Nurse

Education:

Professional Experience:

• 2-4 years of related Hospice experience
• Licensed as a Registered Nurse within the state of operations
