Job summary

Ascend Hospice in the Kansas City area is seeking a dedicated Admissions Nurse (KSMO area) to perform initial patient/family assessments on admission and coordinate home care plans. You will work Tue-Fri, 12pm-10pm, collaborating with the clinical team to outline referrals and care plans. Requires Associate Degree in Nursing; RN license in state; 2-4 years hospice experience.

Bachelor's degree preferred. Competitive salary and benefits are offered, with mileage reimbursement and growth

Qualifications

• Associate's Degree in Nursing required; RN licensure in Kansas.
• 2-4 years hospice experience preferred.
• Bachelor's degree in nursing preferred but not required.

Responsibilities

• Performs initial patient and family assessments upon admission to determine home care needs.
• Outlines patient/family referrals and admission processes.
• Uses assessment data to determine and initiate the plan of care.
• Articulates hospice philosophy, goals and objectives.
• Coordinates with the clinical team to implement the care plan and supports transitions.

Hours: Tuesday - Friday days, 12pm-10pm

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission.
• Ability to outline the patient/family referral and admission.
• Uses assessment date to determine care plan needs and initiates the plan of care.
• Articulates hospice philosophy, goals and objectives.
• Communicates with other members of the clinical team to coordinate the care plan.
• Markets to acute hospitals in the area.

Education

Associate's Degree In Nursing Required, Bachelor's Degree Preferred

Professional Experience

• 2-4 years of related Hospice experience
• Licensed as a Registered Nurse within the state of operations
