Job summary

Advocate Health seeks a part-time Registered Nurse to join the 10422 IL Hospice Divisional - Patient Access team. You will lead the interdisciplinary care team, assess patients for hospice eligibility, and coordinate services across settings to ensure high-quality, patient-centered care.

You will obtain history, explain hospice options, verify prognosis with the IDT, and support billing and communications with families. Travel within the service area may be required.

Qualifications

• Active Registered Nurse license issued by the state of practice.
• BLS certification required for Healthcare Providers.
• Associate degree in Nursing required.

Responsibilities

• Manage admission to hospice services; lead the interdisciplinary care team for patients on caseload.
• Obtain patient history and perform physical assessments to determine eligibility and level of care.
• Explain hospice services and obtain informed consent; verify prognosis with IDT and medical director.
• Review insurance/Medicare eligibility and document data to support billing.
• Coordinate services and communicate with IDT; arrange equipment and supplies as needed.
• Provide hospice education to inpatient hospital and facility staff as directed by manager.
