Job summary

Advocate Health Care is seeking a qualified Hospice Registered Nurse to lead the interdisciplinary care team and manage patient admissions. You will assess patients, explain hospice services, obtain consent, and verify prognosis with the IDT.

The role includes coordinating with multiple disciplines, reviewing insurance eligibility, and arranging equipment and medications to support comprehensive hospice care in the field.

Qualifications

• RN license required in the state of practice.
• Associate's Degree in Nursing is required.
• Typically 2 years of direct patient care experience is expected.
• BLS certification is required or obtainable within 6 months.

Responsibilities

• Responsible for admission to hospice services and leads the interdisciplinary care team for each patient and family.
• Obtains history and performs physical assessments to determine appropriateness for hospice and level of care.
• Explain hospice services to patients and families and obtain informed consent; verify prognosis with IDT.
• Review insurance/Medicare eligibility and document data to support billing.
• Coordinate services with the interdisciplinary team to ensure continuous patient care and arrange equipment and supplies.
