Job summary

Advocate Health seeks a part-time Hospice Divisional - Patient Access RN to lead admission processes, assess patients, and coordinate care with the interdisciplinary team. Qualifications

• Registered Nurse license issued by the state where practicing.
• Associate's Degree in Nursing.
• Typically requires 2 years of direct patient care experience.

Responsibilities

• Responsible for admission to hospice services and leads the interdisciplinary care team for their caseload.
• Obtains history and completes physical assessment; collaborates with physician, medical director and IDT.
• Explains hospice program and obtains informed consent; verifies verbal prognosis with IDT.
• Reviews insurance/Medicare eligibility and documents data for billing.
• Assesses patient/family readiness and develops the plan of care with involvement.
• Coordinates services with IDT; arranges equipment and supplies; ensures handoffs.
• Facilitates placement in hospice contracted facilities when needed.
• Coordinates delivery of medications and home medical equipment.
• Demonstrates effective communication and conflict resolution; promotes collaboration.
• Provides hospice education to inpatient hospital and facility staff as directed.
• Acts as expert resource to hospital/facility staff regarding hospice care.
