Job summary

Advocate Health Care is seeking a skilled Registered Nurse to oversee hospice admissions and lead the interdisciplinary care team for each patient and family. The role includes obtaining histories, performing assessments, and coordinating care across multiple disciplines to ensure appropriate hospice services.

You will educate patients and families about hospice, obtain informed consent, and verify prognostic information with the medical director and IDT.

Qualifications

• RN license in the state where practicing is required.
• BLS certification required; may obtain within 6 months if needed.
• Associate's Degree in Nursing is required.
• Typically 2 years of direct patient care experience.

Responsibilities

• Admit patients to hospice services and lead the interdiscplinary care team for each patient/family.
• Obtain history and perform physical assessment to determine appropriateness for hospice and level of care; collaborate with physicians and IDT.
• Explain hospice services to patients/families; obtain informed consent and verify verbal prognosis with medical director and IDT.
• Review Medicare/insurance eligibility and benefits; document data to support billing.
• Assess patient/family readiness and develop/implement the plan of care with referrals as needed.
• Coordinate services with the interdisciplinary team; arrange equipment and supplies; communicate for handoffs.
• Provide hospice education to inpatient hospital and facility staff as directed by manager.
• Serve as expert resource on hospice and palliative care for hospital staff.
