Qualifications

• BLS for Healthcare Providers required or obtainable within 6 months.

Responsibilities

• Lead the interdisciplinary care team for each hospice patient and family on their caseload.
• Assess patient status and determine eligibility for hospice services and level of care.
• Meet with patients/families to explain hospice services and obtain informed consent.
• Review insurance/Medicare eligibility and assist with billing data.
• Coordinate services with the interdisciplinary team and arrange equipment as needed.
• Provide hospice education to inpatient hospital and facility staff.
