Job summary

Advocate Health seeks an experienced Registered Nurse to coordinate hospice patient and family care across settings. You will educate families on disease trajectories, comfort care, and end-of-life planning while leading the interdisciplinary group to develop and update the Plan of Care.

You will document thoroughly in the EHR, monitor patient responses, and collaborate with physicians, nurses, and social workers to ensure high-quality, compassionate care within Wisconsin and Illinois licenses.

Qualifications

• Requires RN licensure in WI or IL and BLS certification.
• Completion of an accredited nursing program.
• Experience in home health, hospice, or acute care settings preferred.

Responsibilities

• Identify and coordinate patient/family care across hospice settings.
• Educate patients and families on disease processes and end-of-life care.
• Lead the IDG meetings to develop and revise Plans of Care.
• Document in the Electronic Health Record accurately and timely.
