Stella Maris Inc. is seeking an Admissions Nurse (RN) - Home Hospice to join our expanding Home Hospice team in central Maryland.
This full-time, Monday–Friday role focuses on admitting patients to appropriate hospice programs and providing education to families, providers, and discharge planners. Responsibilities include acting as the main contact for hospice services, explaining coverage and payment options, coordinating admissions, and developing initial care plans with input from patients
Qualifikationen
- Maryland RN licensure required.
- EPIC electronic medical record experience preferred.
- Prior hospice or home health experience required.
- Valid driver’s license and reliable transportation.
Aufgaben
- Be the main contact for information about Stella Maris Home Hospice services for patients and families.
- Explain insurance benefits and program coverage, and processing payments or options.
- Coordinate admissions and complete paperwork promptly.
- Develop initial plan of care with input from patient, family, and physician.
- Provide education to physicians, discharge planners, and community organizations about Stella Maris Home Hospice.
- Assume on-call duties as needed.
