Job summary

Bristol Hospice in Oshkosh, WI is seeking an Admissions RN to increase access to hospice services in the communities served by this location. The role focuses on admissions coordination, education of stakeholders, and timely, compassionate patient care.

Responsibilities include coordinating referrals, conducting assessments, and collaborating with ED and DPCS to promote seamless admissions and high-quality hospice support for patients and families.

Qualifications

• Ability to read and follow written instructions.
• Understanding of hospice philosophy and ability to provide specialized hospice care.
• Ability to work autonomously with minimal supervision.
• Demonstrated tact, patience and professional hygiene.
• Strong communication and organizational skills.
• Understanding of plan of care documentation.

Responsibilities

• Relate information to DPCS/ED to promote hospice access.
• Ensure educational needs of the community related to hospice care are met.
• Promote Bristol Hospice as a high-quality provider in the community.
• Coordinate with referral sources to promote timely admissions.
• Complete hospice consults and admissions.
• Provide initial patient/family assessments to determine hospice needs.
• Coordinate with hospice leadership and ED for care planning and education.
