Job summary

Bristol Hospice in Madison, WI is seeking an Admissions RN to establish relationships with customers and referral sources in the community and to expand access to Bristol Hospice services.

The role requires current Wisconsin RN license, ability to travel up to 20%, and flexibility in hours; salary range is $80,000–$95,000 with comprehensive benefits and opportunities for professional development.

Qualifications

• Must have a current RN license in Wisconsin
• Minimum of one year of nursing experience or a BSN
• Must be able to travel 20% of the time
• Must be flexible in work hours
• Willing to maintain knowledge of information systems and software

Responsibilities

• Establish and maintain relationships with customers and referral sources in the community
• Develop and expand access to Bristol Hospice services in the served communities
• Coordinate admissions and ensure timely referrals and information sharing
• Provide professional nursing care and plan of care documentation
• Participate in QAPI processes as directed
• Act as RN case manager as assigned
