Job summary

Bristol Hospice - Madison LLC is seeking an Admissions RN to establish and maintain relationships with customers and referral sources in the community. The role develops access to Bristol Hospice Services and coordinates admissions and care planning.

The position requires an active RN license in the state, travel about 20% of the time, and flexibility in work hours. A BSN or NLN-accredited nursing degree is preferred, with strong communication and organizational skills to support patient and

Qualifications

• Must have a current RN license in the state
• Minimum of one year of experience as a professional nurse within the last three years or a bachelor’s degree in nursing from an NLN-accredited program
• Must be able to travel 20% of the time, often without notice
• Must be flexible in work hours
• Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Responsibilities

• Promote hospice access in the hospice community by coordinating with DPCS, ED and Community Education Director
• Establish and monitor community and payer perceptions of Bristol Hospice as a high-quality provider
• Facilitate hospice referral process and admissions
• Coordinate with hospice agency leadership and referral sources to ensure timely admissions
• Provide professional nursing care using the nursing process and conduct patient assessments
• Participate in IDT meetings and develop patient/family plan of care
