Job summary

Hospice Alliance Inc in Wisconsin is seeking a compassionate Hospice and Palliative Care Registered Nurse to plan and deliver patient-centered care using the nursing process. You will perform admissions visits, complete admission assessments, coordinate with the Case Manager and interdisciplinary team, and provide education to patients and families in home and facility settings.

You will travel to patients' homes, follow the shift coordination, and participate in on-call rotations.

Qualifications

• Must hold RN license in Wisconsin and be eligible to practice in the state.
• BSN preferred for this Hospice and Palliative Care role.
• CHPN certification desired to enhance care delivery.
• Current Basic Life Support (BLS) certification required.

Responsibilities

• Plan and deliver patient-centered care using the nursing process in homes and facilities.
• Conduct admissions visits, assessments, and documentation for care planning.
• Collaborate with the interdisciplinary team and Case Manager for patient needs.
• Provide education to patients and families about hospice care and medications.
• Coordinate with physicians, NP, and social workers for ongoing care.
