Advocate Health is seeking a Hospice Admissions Nurse to join our Chicago Metro hospice team. This full-time, 32-hour/week role features a 2:00 PM–10:00 PM schedule with Monday, Tuesday, Thursday, and Friday shifts, no on-call requirements and travel between locations.

The ideal candidate is a compassionate registered nurse with strong assessment and communication skills, capable of coordinating an interdisciplinary team and supporting families through the admission process.

Qualifications

Qualified RN with compassionate bedside manner.
Strong assessment and communication skills for hospice admissions.
Able to coordinate with interdisciplinary team and families.

Responsibilities

Lead the interdisciplinary team for each patient on caseload.
Obtain history and perform physical assessments to determine hospice eligibility.
Explain hospice services and obtain informed consent for care.
Verify insurance/Medicare benefits and support billing.
Coordinate services, equipment, and home medical supplies.
Facilitate appropriate placement in contracted hospice facilities.
Deliver hospice education to hospital and facility staff.
Act as a resource on hospice and palliative care for teams.
