Aufgaben

• Lead the interdisciplinary care team for each patient and family
• Perform histories and physical assessments and determine eligibility for hospice care
• Explain hospice services and obtain informed consent; verify prognosis with IDT
• Review insurance/Medicare eligibility and document for billing
• Coordinate services and equipment with the care team; ensure smooth handoffs
• Assist with placement in contracted hospice facilities when needed
• Coordinate medications, supplies, and home medical equipment
• Provide hospice education to hospital and facility staff as directed
• Serve as a resource on hospice and palliative care
