Job summary

Sangre de Cristo Community Care is seeking a compassionate Hospice Admissions Nurse in Southern Colorado to be the first point of contact for patients and families beginning their hospice journey. You will guide the process with care, empathy, and expertise as they start this path.

This full-time role (Tuesday-Saturday) includes competitive pay, mileage reimbursement, comprehensive benefits, and a supportive interdisciplinary team focused on comfort and dignity.

Qualifications

• Must have a valid non-restricted RN license in Colorado.
• Must be at least 21 years old and have a valid driver’s license.
• Flexibility to work a weekend schedule if needed.
• At least 1 year of hospice or home health experience preferred.

Responsibilities

• Explain hospice care to patients and families with empathy and clarity.
• Conduct patient assessments, obtain consents, and initiate care plans.
• Collaborate with the interdisciplinary team to ensure a smooth transition into care.
