Sangre de Cristo Community Care - Hospice in Colorado is seeking a compassionate Registered Nurse to join as a Hospice Admissions Nurse, the first point of contact for patients and families beginning their hospice journey.

You will guide them with care, empathy and expertise, perform initial assessments, obtain consents, initiate care plans, and collaborate with the interdisciplinary team to ensure a smooth, supportive transition into care. This full-time role runs Tuesday through Saturday.

Qualifications

• Active Colorado RN license required.
• Non-restricted driver's license.
• 21+ years old.
• Weekend availability if needed.
• 1 year hospice or home health experience preferred.
