Job summary

Sangre de Cristo Community Care is seeking a Float RN to provide clinical support across our program. The role includes admissions, patient visits, and coverage for RN Case Managers during PTO and staffing needs, ensuring continuity of care across the team.

Ideal candidates will have at least one year of nursing experience, with hospice/home health or related fields preferred, strong assessment skills, and a patient-centered approach. Competitive benefits and growth opportunities are offered.

Qualifications

• Minimum one year of nursing experience required.
• Hospice, Home Health, Oncology, Palliative Care, or ICU experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Ability to adapt quickly to changing priorities and patient assignments.
• Compassionate, patient-centered approach to care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Completing hospice admissions and start-of-care visits.
• Providing routine and PRN patient visits.
• Covering RN Case Manager caseloads during PTO/absences/education.
• Assisting with symptom management and changes in patient condition.
• Collaborating with physicians and the interdisciplinary team to develop and update care plans.
• Educating patients and families on disease progression, medications, and end-of-life care.
• Supporting timely response to changing patient needs.
• Documenting all patient care in the EMR.
• Maintaining continuity of care across the team.
