Job summary

Lifted Hospice is seeking an Admissions Nurse to welcome patients and families into hospice with clarity, compassion, and clinical excellence. You will guide transitions, complete initial assessments, and collaborate with the care team to create individualized plans of care from day one.

You will work closely with nursing leadership and the interdisciplinary team, travel locally for admissions, and participate in on-call rotations.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor's nursing program with an active state license.
• Two years of RN experience in a clinical care setting; hospice experience preferred.
• Strong assessment, critical thinking, and communication skills for urgent conversations.
• Familiarity with state and federal hospice regulations and CoPs.
• Ability to work independently, prioritize, and make decisions under pressure.
• Must be able to travel locally to admissions across homes and facilities.

Responsibilities

• Assessing patient needs at admission with timely, thorough nursing assessments.
• Build the plan of care in coordination with patient, family, and interdisciplinary team.
• Provide skilled nursing care at start of care with infection control per OSHA standards.
• Collaborate with medical director, physicians, leadership, and team for admitted patients.
• Submit accurate admission documentation and ensure regulatory compliance.
• Support families with education and clear communication about the hospice journey.
