Job summary

Lifted Hospice is seeking an Admissions Nurse (RN) to welcome patients and families into hospice with clarity and compassion. You will conduct initial assessments, provide skilled nursing care, and help families understand the care plan from day one.

You will collaborate with the full care team to ensure a smooth transition, document thoroughly, and support patients and families through the admissions process with dignity and professionalism.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program with active state license.
• Strong assessment and clinical decision-making abilities are required.
• Excellent communication skills for sensitive discussions with patients and families.
• Ability to travel locally to complete admissions.
• Familiarity with state/federal hospice regulations and CoPs.
• Ability to work independently and manage time under pressure.

Responsibilities

• Assessing needs at admission with comprehensive nursing assessments.
• Building the plan of care in collaboration with patient, family, and interdisciplinary team.
• Providing skilled nursing care and ensuring safety and infection control.
• Collaborating with medical director, physicians, hospice leadership, and team.
• Maintaining accurate, timely admission documentation and records.
• Educating and supporting families to understand the hospice journey.
• Participating in on-call rotations and after-hours care as needed.
• Engaging in ongoing training and quality improvement initiatives.
