Sangre de Cristo Community Care is seeking a compassionate Hospice Admissions Nurse in Southern Colorado to be the first point of contact for patients and families beginning their hospice journey. You will guide the process with care, empathy, and expertise as they start this path.

This full-time role (Tuesday-Saturday) includes competitive pay, mileage reimbursement, comprehensive benefits, and a supportive interdisciplinary team focused on comfort and dignity.

Qualifikationen

• Must be at least 21 years old and have a valid driver’s license.

You'll help guide them with care, empathy, and expertise, ensuring they feel supported, informed, and comforted every step of the way.
