Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse to deliver compassionate, person-centered home-based hospice care. You will assess patients, develop and direct individualized care plans with the IDG, and ensure quality nursing care in the home setting.

Required: RN license, BLS within 30 days, and preferred hospice experience. Expect on-call duties, thorough documentation, and collaboration with a multidisciplinary team to support patients and families with dignity at end

Qualifications

Current, unencumbered RN license in the state of employment.
BLS certification within 30 days of hire (American Heart Association).
Preferred hospice nursing experience (1 year).

Responsibilities

Completes assessment of patient and family to determine Hospice needs.
Writes and initiates Plan of Care in collaboration with the IDG.
Regularly reevaluates patient/family/caregiver needs and revises the Plan of Care.
Develops care plan with patient/family/IDG based on nursing diagnosis.
May administer medications and treatments as prescribed by the IDG Plan of Care.
Participates in on-call duties per policy.
Documents accurate clinical notes and information management.
Supervises ancillary staff and participates in orientation and education.
