Job summary

Providence at Home with Compassus is hiring a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in patients' homes. You will assess needs, provide professional nursing care, and work with the Interdisciplinary Group to plan and direct individualized care plans.

Qualifications

• Current RN license in the state of employment.
• National Provider BLS certification (American Heart Association) within 30 days of hire.

Responsibilities

• Complete assessments of patient and family to determine hospice needs.
• Develop and initiate Plan of Care in collaboration with the Interdisciplinary Group (IDG).
• Regularly reevaluate patient/family needs and revise the Plan of Care as needed.
• Administer medications and treatments per the approved Plan of Care.
• Participate in on-call duties per procedures.
• Maintain accurate clinical notes and document care provided.
